Practice English any and every chance you get!
Watch American movies in their original language - without voice-overs. Turn on the subtitles the first few times you watch the movie until you increase your confidence
Watch English programs on TV with the subtitles turned on (a TV setting)
Watch youtube videos - anything in English that interests you (It must be something that interests you! Even music videos will help you improve.)
Join any of the numerous online communities to chat
Read magazine or newspaper articles which interest you. (Don't suffer through trying to read articles which don't.) Learn the particular jargon for your interests.
Remember: everybody has trouble communicating when they are learning a language sometimes. It's all in your attitude!
